General Agreement Statement and General Health Questionnaire

Dear Customer, Certain underlying health conditions need to be taken into consideration to ensure that you receive the most appropriate service. Therefore, it is necessary for us to gather information about your health status. For children, we kindly ask parents/guardians to fill out the questionnaire. All information received from you, like any other health data, will be treated strictly confidentially in accordance with legal regulations. Please be informed that in the case of certain illnesses, we may refuse participation in classes or sessions. These may include conditions such as epilepsy, chronic inflammatory diseases in the eye, ear, nose, and throat area, acute illnesses, fever, high blood pressure, pregnancy, etc. If you suffer from any chronic illness, please consult your general practitioner, and kindly inform our instructors in advance. General Consent Form

I have read and understood the Rules and Regulations of Aerial Yoga Hungary and Aerial Sports Academy. I acknowledge that I am responsible for any damage or accidents resulting from non-compliance with the rules, and the improper use of equipment and facilities at Aerial Yoga Hungary and Aerial Sports Academy. I do not hold Aerial Yoga Hungary and Aerial Sports Academy or its staff responsible for injuries resulting from my negligence. I understand that Aerial Yoga Hungary and Aerial Sports Academy do not provide a refund guarantee. The purchased service amount is non-transferable and cannot be redeemed for another service. Passes cannot be extended and must expire within the specified time frame. To the best of my knowledge, I do not have any illnesses or complaints beyond those specified in the General Health Questionnaire. I have filled out the questionnaire truthfully, and I do not have any undisclosed illnesses. I consent to Aerial Yoga Hungary and Aerial Sports Academy recording and maintaining information about my health status, in accordance with the relevant legal provisions, and to be informed about classes and sessions at the provided contact addresses. My data will not be shared for commercial purposes. I agree to receive notifications from Aerial Yoga Hungary and Aerial Sports Academy regarding class schedules and changes at the provided contact addresses. I am aware that smoking is prohibited throughout the facility. If I violate this prohibition, I commit to paying the imposed fine. I understand that if I cannot attend a pre-booked session, I must cancel it at least 24 hours in advance. For cancellations within 24 hours, a penalty fee is payable, equal to the cost of the booked service. Cancellations within 24 hours will also be deducted from the passes of our pass holders. Daily pass holders will be charged a penalty equal to the daily pass amount, which must be settled on the next visit. Failure to pay the penalty will result in the participant being unable to attend classes. I acknowledge that for Aerial Yoga Hungary and Aerial Sports Academy classes, I must arrive 15 minutes before the scheduled start time, and there is no possibility to join the class after it has started, for safety reasons. I consent to the possibility of photographs/videos being taken during Aerial Yoga Hungary and Aerial Sports Academy classes, which will be uploaded to a closed Facebook group created for this purpose. Only the instructor can take photos and videos during the classes, with the participant's consent. If I do not agree, I will notify the instructor at the beginning of the class and indicate it in the consent form. I made these decisions freely, without any coercion, after receiving personalized answers to all my questions and understanding them. I acknowledge that I have received all necessary information for my informed consent and have had sufficient time to consider it.
